• Valid only with the Maybank Philips Zoom Teeth Whitening Package for Maybank Card members. Any optional test(s) are upon request and will be charged separately.
• Payment must be made with a Maybank Visa or Mastercard (Malaysia, Singapore, lndonesia, Philippines, Brunei and Cambodia).
• Any follow-up investigation or treatment or medication as a result of this screening will be charged by cash, debit or credit cards.
• Available only to individuals who are independently mobile.
• Consultation/ Procedure for Philips Zoom Teeth Whitening Package will be by Dr. Khaw Ying Ying or Dr. Nicholas Ong Jia Weh.
• Appointments must be made at least three (3) working days in advance and are subject to availability. Call 04-238 3388 ext 2698 to make an appointment.
• Payment is to be made on appointment day and is non-refundable and cannot be redeemed for cash nor any other items and services
• The decision is made by Island Hospital is final and non-negotiable.
• Island Hospital reserves the rights to refuse any requests without giving reasons or change the terms and conditions or cancel any promotions at any time without notice.
• Maybank makes no representation as to the quality, merchantability or the fitness for any purpose of the goods and/or service(s) provided by Island Hospital.
